There are two main eye related problems which can occur when using computers in offices, dry/itchy eyes, and tired eyes.

This assessment is for TIRED eyes, for an assessment of dry/itchy eyes, please follow this link. Each box is a NO answer.

1.) If so, does the duration of these tasks mean you don't need a document holder (e.g. 20 minute stints)?
2.) Can documents be reviewed with the head in a balanced posture?
3.) If no, has a document holder provided?
4.) If yes, is the document holder of a suitable size and position?
5.) Is there enough space on the desktop for the flow of work?
6.) Is the layout of the desktop equipment satisfactory?
7.) Is the chair comfortable?
8.) Is the top of the visible area of the screen at eye height when you are sitting upright?
9.) Is the monitor at a suitable distance from your eyes (20"-26")?
10.) Is the information on the screen well defined and easy to read?
11.) Can you adjust the brightness and contrast easily?
12.) Is the image stable and flicker free?
13.) Is the screen free from glare and reflections?
14.) Does the layout of the immediate work area allow the job to be done in a correct posture?
15.) Is there adequate lighting for all the tasks?
16.) Is the background behind the screen slightly less bright than the screen?
17.) Is the temperature comfortable for your, most of the time?
18.) Is humidity normally comfortable for your eyes and sinuses?
19.) Are you free from stressing levels of uncertainty and anxiety when using software?
20.) Have you received information on how to avoid visual fatigue?
21.) Have you been made aware of ways to detect and avoid stress at work?
22.) Have you received information about the provision of eyesight tests and corrective lenses?
23.) Have you been informed about taking breaks and changes of work activities?
24.) Have you had training and information in how to adjust your workstation?
