This questionnaire will help you resolve any pain in your buttocks (ischia). Each box is a NO answer.

1.) Is there enough space on the desktop for the flow of work?
2.) Is there adequate leg room (height, width and depth)?
3.) Is the layout of the desktop equipment satisfactory?
4.) Is the chair comfortable?
5.) With the seat and desk adjusted correctly for the elbows, are the feet firmly on the floor without compressing the underside of the thighs?
6.) If a footrest is (thus) required, is an appropriate one present?
7.) If not, are you going to get one without a Health & Safety intervention?
8.) Is the top of the visible area at eye height when you are sitting upright?
9.) Is the monitor at a suitable distance from your eyes (20"- 26")?
10.) Is the monitor directly in front of you so that you do not sit twisted?
11.) Is the information on the screen well defined and easy to read?
12.) Can all of your tasks be completed without the need to refer to paper documents?
13.) If so, does the duration of these tasks mean you don't need a document holder (e.g. 20 minute stints)?
14.) Can documents be reviewed with the head in a balanced posture?
15.) If no, has a document holder provided?
16.) If yes, is the document holder of a suitable size and position?
17.) Is the extra equipment located in a position that is compatible with correct posture?
18.) Is there space to manoeuvre the chair?
19.) Does the layout of the immediate work area allow the job to be done in a correct posture?
20.) Have you been shown how to adopt good posture at the workstation?
21.) Have you received information on how to avoid visual fatigue?
22.) Have you been made aware of ways to detect and avoid stress at work?
23.) Have you received information about the provision of eyesight tests?
24.) Have you been given information about the provision of corrective lenses?
25.) Have you been informed about taking breaks and changes of work activities?
26.) Have you had training and information in how to adjust your workstation?
